LifeLinks Care Management & Advocacy
- Senior Services
Our mission at LifeLinks is to change the aging process from one of fear, institutionalization, and confusion to one of laughter, reconciliation, and good memories.
Caring for a loved one or individual that is aging or has disabilities can be overwhelming. As the people we care for begin to age it may become increasingly difficult and stressful to manage doctor's appointments, medical bills, information, transportation, and care for them. Many families recognize the need for help, but have no idea where to start. At LifeLinks, our care team can help provide answers to some of the questions. We assess each family's situation, meet with the individual, and develop a care plan that is right for the them and the family. We act as representatives for the family, and work with them to provide solutions that offer loved ones the care and quality of life they deserve.
A patient advocate is singularly focused on health concerns, where as LifeLinks sees the whole person. Health is only one aspect of a person’s life.
